If you already have a paper-based survey that you would like to convert to an on-line tool, send it to us. We will put it on-line for you and show you how to get it out to your target audience.

We prefer to start with an electronic version of your questionnaire in MS Word but if it is on paper only, you can fax it to us.

Once you have selected the questionnaire that you would like us to convert to an on-line tool, take the following steps...

STEP 1. Send your contact information and a copy of your questionnaire to attached to an e-mail to al_holmes@mts.net or fax it to us at 204-389-4798.

STEP 2. Once we have made arrangements for payment, we will convert your document to an on-line survey and send you a link to it so you can look it over. Once you are happy with it, we will help you launch it. This means you will use this link to invite participants to respond via e-mail or a link on your website. We will also send you another link so that you can access survey results as they come in.

We usually can "Load and Launch" one of your surveys for $1,000.00 or less. This price includes providing a link that you can e-mail to respondents or place on a website, and a second link that gives you direct access to the survey results as they arrive.

We can even send out your new online questionnaires to a mailing list of e-mails that you provide and track responses for you and send reminders at pre-determined dates. We can give you a quote on this extra service.
