Sequus is a pioneer in the use of surveys to assess training needs. These surveys began more than 25 years ago and have evolved into their current on-line format from client feedback.

GENERIC FINANCIAL MANAGEMENT SKILLS SURVEY We have developed several needs assessment surveys for specific occupations. We begin with a list of competencies either provided by the client or developed by us through a DACUM process. If we already have one that works for you, you can use it. If we have to develop a new one, contact us for prices.
TRAINING/LEARNING EVALUATION SURVEYS
Sequus is a supporter of the 5-level Kirkpatrick model of training evaluation. We use on-line surveys to collect input from participants, their supervisor/manager and others. Here are samples for each level.
Level One Participant Reaction: This is used to measure how participants feel about the training. Are they satisfied or disatisfied? As the consumer, this is a key evaluation component, especially for adult learners.
Level Two Participant Knowledge-Skill Change: This is the level where we test the learners' knowledge acquired in the training. This is closest to an exam. a good design will have a pre and post test. Since there is such a broad range of subject matter out there it is difficult to develop a generic example. The link takes you to a pre-test for one of our entrpreneurship workshops.
At this level, we meaure what the learner is doing differently back on the job. The questionnaire is often completed by persons in a position to observe the behaviour change such as a supervisor or manager, or a colleague, subordinate, client or supplier.
Level Five: Return on Investment This level of evaluation tries to measure the economic value of the impact and compare it to the cost of delivering the training. Is there a return on the training investment? The formula is COST OF TRAINING/(VALUE - COST OF THE TRAINING). While we don't have a survey for this calculation, the link will take you to a spreadsheet that will help.
SURVEYS FOR NON-PROFIT ORGANIZATIONS
Sequus has provided services to the non-profit sector around the world for decades.
THE MEMBER SERVICES USER FEEDBACK SURVEY This survey is specially designed for those organizations that provide services to their members. Typically these organizations are formed around a profession or trade. This survey is typically used to get feedback from members on the services provided.
This survey is similar to the one above but allows staff, Board and others to assess the performance of a member services organization.
THE CLIENT SATISFACTION SURVEY This survey allows you to measure the level of client satisfaction with your organization and set goals for improvement.
SUPPORTER SURVEY This survey is specially designed for those organizations that raise funds from a large base of supporters.

LEADERSHIP SURVEYS
Sequus has developed several surveys for leadership self-assessment. Because they take a little more explanation and more work in the diagnosis they don't quite fit the 'ready-to-go' category. You can learn more about them at our new leadership website.
Once you have selected a standard on-line survey that you would like to use, take the following steps...
STEP 1. Send your contact information and the name of the on-line questionnaire you would like to use to al_holmes@mts.net or give us a call at 204-389-4149.
STEP 2. Once we have made arrangements for payment, we will launch your survey, which means we will send you a link to your version of the "Ready-to-Go" on-line survey. You will use this link to invite participants to respond via e-mail or a link on your website. We will also send you another link so that you can access survey results as they come in.
Most of our "Ready-to-Go" on-line surveys can be launched for $300.00 or less. This price includes providing a link that you can e-mail to respondents or place on a website, and a second link that gives you direct access to the survey results as they arrive.
